AP Govt Employees Pensioners Health Card Enrollment Application Form

AP State Government Employee, and Pensioner can download the Mee Seva health card enrolment application form from the official website www.ehf.gov.in. The enrolment application form has the following information. All employees and pensioners need to fill the application form and submit the same to the heads of their departments.

1. Employee Code

2. Aadhar Number/Aadhar Enrolment Receipt Number

3. Personnal Details

sex, community, marital status, date of birth, date of joining in the service, whether disabled?, residential address, Email, mobile number, office address,

4. Identification Details

Ration Card Number, Identification Mark1 and Identification Mark2

5. Current Posting Details or Last Posting Details (In case of Pensioner)

Head of the department, DDO Code, Category, Name of the district, Pay grade, Pay Scale, current pay, Date of last posting, STO/APPO Name, STO/APPO Code


1.Service Register 2 Pages


3.Aadhar Number/Aadhar Receipt

4.Date of Birth Certificate

The above details have to be filled by the employee/pensioner. They have to submit the filled in application form to their respective head of the departments
